Postabortal Group A Streptococcal Sepsis and Toxic Shock Syndrome
نویسنده
چکیده
A 20-year-old woman developed lower abdominal pain and fever within 48 h of undergoing an elective termination of a 10 week pregnancy by suction dilation and curettage. Progressive clinical deterioration prompted an exploratory laparotomy which revealed necrotic ovaries bilaterally (A). Areas of the myometrium were also ne-crotic and hemorrhagic (B). An abdominal hysterectomy and bilateral salpingo-oophorectomy was performed.

The contribution of group A streptococcal virulence determinants to the pathogenesis of sepsis
Streptococcus pyogenes (group A streptococcus, GAS) is responsible for a wide range of pathologies ranging from mild pharyngitis and impetigo to severe invasive soft tissue infections. Despite the continuing susceptibility of the bacterium to β-lactam antibiotics there has been an unexplained resurgence in the prevalence of invasive GAS infection over the past 30 years. Of particular importance...
متن کاملTwo cases of delayed diagnosis of postpartal streptococcal toxic shock syndrome.
BACKGROUND Puerperal sepsis due to group A beta-hemolytic streptococcal (GAS) toxic shock syndrome is associated with very high morbidity and mortality. Luckily it is now rare, but diagnosis is not always easy. This report demonstrates the problem of recognizing this disease, and summarizes the current knowledge on the pathomechanism and management of streptococcal toxic shock syndrome. CASE ...

We herein report the case of a 66-year-old woman presenting with symptoms of gastroenteritis. Computed tomography showed small-bowel dilation without ischemic signs. After admission, she went into shock and was treated for sepsis of unknown origin. She was later diagnosed with group A streptococcal peritonitis due to an ascending vaginal infection. This case highlights the importance of conside...
متن کاملGroup A streptococcal puerperal sepsis with retroperitoneal involvement developing in a late postpartum woman: case report.
Group A beta-hemolytic streptococci cause a wide range of infectious diseases such as pharyngitis, impetigo, rheumatic fever, and even septic shock. Group A streptococcal puerperal sepsis is uncommon today, but recent reports indicate a reemergence of virulent strains can cause toxic-mediated shock and multiple organ failure. We present a case report of a 29-year-old postpartum woman after cesa...

Since the mid-1980s, concern has grown that invasive group A Streptococci (GAS) has been increasing in incidence and severity. Invasive infections caused by group A Streptococcus or Streptococcus pyogenes include sepsis, arthritis, pneumonia, meningitis, necrotizing fasciitis (NF) and streptococcal toxic shock syndrome (STSS). GAS also causes noninvasive suppurative disease as pharyngitis and o...
